This program allows eligible individuals to access Medicare rebates for essential allied health services, including osteopathy, to better manage their condition and improve their quality of life.

What is the Medicare Chronic Disease Management (CDM) Plan?

The Medicare CDM Plan, also known as the Enhanced Primary Care (EPC) Program, is designed to assist patients with chronic medical conditions by providing access to a range of allied health services. These services aim to address and manage the impact of chronic illnesses on individuals’ day-to-day lives.

Who Qualifies for the CDM Plan?

To qualify for the CDM Plan, you must meet specific criteria set by Medicare which includes:If you have a chronic medical condition (has or is likely to last 6 months or more) which include pain or injury.

How Does Osteopathy Fit into the CDM Plan?

Under the CDM Plan, eligible patients can receive Medicare rebates for up to five allied health visits per calendar year. This includes osteopathy services, which focus on treating musculoskeletal issues and promoting overall wellness through manual therapy techniques. Currently the Medicare rebate is $60.35.   

 How to Access Osteopathy Services Under the CDM Plan:

1. Consult Your GP: Schedule an appointment with your GP to discuss your chronic health condition and the suitability of a Chronic Disease Management Plan.
2. Prepare a Care Plan: If your GP determines that you qualify for the CDM Plan, they will work with you to develop a personalized Care Plan that outlines your healthcare needs and goals.
3. Referral to Osteopathy at Rapid Recovery: Your GP will provide you with a referral to an osteopath as part of your Care Plan. This referral is essential to access Medicare rebates for osteopathy services & to use with us must be made out to Rapid Recovery. 
4. Book Your Osteopathy Appointments: Contact us to schedule your appointment or you can book online.
