What is the Medicare Chronic Disease Management (CDM) Plan?
The Medicare CDM Plan, also known as the Enhanced Primary Care (EPC) Program, is designed to assist patients with chronic medical conditions by providing access to a range of allied health services. These services aim to address and manage the impact of chronic illnesses on individuals’ day-to-day lives.
Who Qualifies for the CDM Plan?
To qualify for the CDM Plan, you must meet specific criteria set by Medicare which includes:If you have a chronic medical condition (has or is likely to last 6 months or more) which include pain or injury.
How Does Osteopathy Fit into the CDM Plan?
Under the CDM Plan, eligible patients can receive Medicare rebates for up to five allied health visits per calendar year. This includes osteopathy services, which focus on treating musculoskeletal issues and promoting overall wellness through manual therapy techniques. Currently the Medicare rebate is $60.35.
How to Access Osteopathy Services Under the CDM Plan:
